Operational Loss Data Collection: Building a Loss Event Database That Satisfies Examiners and Feeds Your Risk Program

Your RCSA rates payment processing controls as strong. Your operational risk committee has seen those ratings for three years running without significant challenge. Then an examiner pulls your loss event database during a review and finds fourteen wire fraud incidents from the past 18 months — none of which made it into your RCSA, none of which triggered a KRI threshold review, and none of which resulted in documented control improvements.
That’s not a loss data problem. That’s an operational risk program that’s been producing false assurance.
Loss event data collection is the piece of ORM that most institutions treat as optional, or build as a compliance checkbox with minimal investment. It’s the only mechanism that can validate — or invalidate — every control effectiveness rating in your RCSA. Without it, your risk appetite statements are opinions. Your three lines of defense model is a governance structure with no feedback loop. Your KRIs are monitoring for risk you’ve never confirmed you’re experiencing.
Here’s how to build a loss event database that actually works.

Why Loss Data Matters Even When Basel Capital Rules Don’t Apply to You
Let’s address the elephant in the room. The Basel Committee’s Standardized Measurement Approach (SMA) for operational risk capital — which formally uses internal loss data through the Internal Loss Multiplier (ILM) — applies primarily to the world’s largest banks. The US Basel III Endgame re-proposal (March 2026) applies only to Category I and II bank holding companies and, notably, pegs ILM to 1, meaning internal loss history won’t directly affect capital requirements for US banks even if they’re in scope.
That makes loss data a Basel capital non-issue for the overwhelming majority of US banks and virtually all fintechs.
It does not make loss data collection optional.
OCC and Federal Reserve examination standards — codified in the OCC’s Sound Practices to Strengthen Operational Resilience guidance and the interagency operational resilience paper — expect institutions to collect and analyze operational loss data as part of a sound operational risk management framework. Loss data is how examiners calibrate whether your RCSA and risk appetite are connected to reality. Without it, examiner conversations about your operational risk profile rest entirely on your representations about your own controls — which is not a comfortable position during a targeted review.
What Counts as a Loss Event
A loss event is any incident that results in a financial loss due to inadequate or failed internal processes, people, or systems, or from an external event. The definition comes from Basel’s foundational operational risk framework — Basel II/III Annex 9 — and it deliberately excludes strategic risk and reputational risk, while including legal risk.
Gross loss is the full monetary impact before recoveries. Insurance proceeds, vendor reimbursements, and recovered funds are tracked separately as recoveries. You report gross loss, not net. A $2 million fraud event where insurance covered $1.8 million was still a $2 million loss event — understating it to $200,000 distorts your risk profile and misleads anyone relying on the data to assess control effectiveness.
Loss events include:
- Direct costs: write-offs, charge-offs, provision increases
- Settlement or judgment payments
- Regulatory fines and penalties
- Legal fees associated with the event
- Cost of remediation (system fixes, process reengineering directly attributable to the event)
- Lost revenue where directly linked to the event (e.g., system downtime preventing transactions)
Loss events exclude:
- General business costs not tied to a specific failure
- Opportunity costs
- Cost of risk management infrastructure unconnected to a specific event
The 7 Basel Event Type Categories — With Financial Services Examples
Using the Basel taxonomy ensures your data is consistent over time, legible to examiners, and benchmarkable against industry loss data. These are Level 1 categories; each has Level 2 sub-categories for more granular classification.

Most financial services loss events cluster in External Fraud (EF), Clients/Products/Business Practices (CPBP), and Execution/Delivery/Process Management (EDPM). If your database shows events uniformly distributed across all seven categories, something’s probably wrong with your classification — or someone’s filtering before events reach the database.
Setting Your Loss Capture Threshold
The threshold question is less about the number and more about consistency.
A $10,000 mandatory reporting threshold is common and practically defensible. Some institutions capture all events regardless of dollar amount, flagging those above $50,000 or $100,000 for detailed analysis. Some use lower thresholds for specific event types — particularly Internal Fraud and CPBP events — where even small incidents signal systemic issues that dollar-threshold filtering would hide.
What examiners challenge is inconsistency:
- Business lines that apply the threshold differently (“we didn’t capture that because it was under $10K, but actually it was $12K — I’m not sure why it’s not in the database”)
- Event types that consistently fall just below threshold because nobody wants to report them
- Ad-hoc exceptions based on who the supervisor of the business line is
Document your threshold in policy. Enforce it uniformly. Review annually whether it’s still appropriate given your transaction volumes, risk profile, and regulatory expectations.
The Fields Every Loss Record Needs
Lean databases fail because they can’t answer the questions examiners ask. Every loss event record should include:
Identity and classification
- Unique event ID
- Event discovery date and event occurrence date (these are often different)
- Business line / cost center where the event originated
- Basel Level 1 event type category (and Level 2 if you use sub-categories)
- Brief description of what happened
Financial data
- Gross loss amount
- Estimated loss at discovery (before final determination)
- Recovery amounts (insurance, vendor reimbursement, other)
- Net loss after recoveries
Status and disposition
- Event status (open / closed / contested)
- Date closed
- Settlement or resolution terms if applicable
- Regulatory notification made? (Yes / No / N/A)
- SAR filed? (Yes / No / N/A)
Root cause and controls
- Primary root cause category (people / process / systems / external)
- Narrative root cause description
- Control(s) that failed or were absent
- RCSA risk category affected
Remediation
- Remediation action taken or planned
- Owner of remediation
- Target completion date
- Closed / open
Near-Miss Events: The Data You’re Not Collecting
A near-miss is an incident with zero (or de minimis) financial impact but meaningful potential for a material loss. The wire payment queued to the wrong account that was caught by a secondary reviewer before processing. The system configuration error discovered before it was exploited. The credit decision made without required underwriting documentation, caught before funding.
Near-misses are more valuable than realized losses for identifying systemic control weaknesses — because you can act on them before the loss materializes. A single $500,000 fraud event tells you a control failed once. Fifty near-miss events in the same risk category over 12 months tells you the control is failing systematically and you’re getting lucky.
Capture near-misses in a separate register (not mixed into your primary loss database, which should reflect actual financial impact). Use the same Basel taxonomy for classification. Review near-miss trends quarterly alongside loss event trends. A rising near-miss frequency in a category where RCSA rates controls as strong is a red flag that deserves immediate investigation.
Governance: Who Owns Collection, Challenge, and Escalation
Loss data quality degrades without clear ownership. The Three Lines of Defense model applies directly:
First line (business units): Own event identification and initial entry. They’re closest to the events and have the context for accurate description and root cause. But first-line capture creates a conflict of interest — nobody wants to report losses that reflect poorly on their controls. Counter this with policy-mandated reporting timelines, no-reprisal language, and second-line challenge.
Second line (Operational Risk / Risk Management): Owns data quality challenge. This means reviewing new entries for completeness and accurate classification, challenging root cause descriptions that are superficial (“human error” without specifying what process failed), and reconciling the loss database against other data sources (General Ledger write-off reports, legal settlement logs, regulatory penalty notices) to identify gaps.
Third line (Internal Audit): Validates the completeness and accuracy of the program periodically — typically annually. Audit scope should include both the database itself and a sample of potential loss events from operational systems (GL write-offs, customer complaint resolution records, system outage logs) to test whether first-line capture is actually capturing events or only those someone chose to report.
Escalation: Define thresholds for escalation to the Risk Committee and Board. A single event above $250,000 (or whatever threshold fits your size) should trigger notification to the Risk Committee within a defined timeframe. Patterns — three or more events in the same risk category within a quarter — should trigger a formal root cause review regardless of individual dollar amounts.
Connecting Loss Data to Your RCSA and KRI Program
This is where most programs fall apart. Loss data collected in isolation doesn’t improve risk management. The connection to your RCSA and KRI program is what makes the investment worthwhile.
RCSA feedback loop: Every material loss event should trigger a review of the RCSA risk rating for the relevant risk category. If External Fraud / card fraud produced three events in a quarter and your RCSA rated card fraud controls as “Adequate,” that rating needs to be revisited. Schedule a quarterly reconciliation: pull losses by Basel event type, map to RCSA risk categories, identify discrepancies between control ratings and observed loss frequency.
KRI calibration: Loss data should inform KRI threshold setting. If your External Fraud KRI threshold is set at “5 card fraud events per month before escalation” and your historical loss data shows that threshold gets hit three times a year, the threshold is too high to be useful as an early warning indicator. Set thresholds based on observed historical frequency, not intuition.
Trend reporting to senior management: Quarterly loss data reporting should include: loss events by event type, total gross and net loss amounts, comparison to prior quarter and prior year, any events above individual materiality thresholds, and near-miss trends. This is distinct from incident-by-incident reporting — it’s an aggregate view designed for risk committee oversight.
Common Examiner Findings on Operational Loss Programs
After years of OCC and Federal Reserve examination commentary on operational risk programs, certain findings recur:
No database at all. Smaller institutions sometimes track losses informally in spreadsheets maintained by individuals, with no institution-level aggregation or governance.
Inconsistent capture across business lines. Some lines report rigorously; others don’t. The result is a database that reflects reporting culture more than actual risk profile.
Shallow root cause descriptions. “System error” and “human error” are not root causes. Examiners push for specificity: which system? Which process step? Which control was absent?
No connection to remediation. Events are logged and closed without documenting what changed. The database becomes a historical record with no preventive value.
Loss data disconnected from RCSA. The RCSA team and the loss data team operate independently, with no reconciliation. Control ratings don’t reflect loss history; loss history doesn’t trigger rating reviews.
So What?
An operational loss database isn’t a regulatory deliverable. It’s the evidence layer that determines whether your RCSA control ratings mean anything, whether your KRI thresholds are calibrated to reality, and whether your risk appetite statement reflects actual experience or aspirational thinking.
The institutions that build effective loss programs treat collection as an operational process — not a compliance project. Events are captured at the business line, challenged by second-line risk, connected to root cause, and fed back into every other risk management process. Examiners know the difference between a database that’s maintained to satisfy a policy requirement and one that’s actually informing risk decisions. So does anyone who reviews your risk committee materials and notices that loss trends never seem to affect control ratings.
